To begin your Team Seattle Individual Membership, we invite you to fill out the form below. You now have the option of paying on-line using PayPal, in addition to paying by check. PayPal securely accepts credit card transactions at no cost to you and it is FAST.

Alternatively, you can print out a PDF of this same form by clicking here. You can, then, mail it in with your $10.00. Student rate is $5.00. A sustaining membership is $40.00 until 12-31-2009.
